While we don’t accept insurance directly, you may be able to submit to your insurance for a reimbursement.
Additionally, all Square receipts will include the CPT code, Tax ID #, and NPI #, as this information is frequently requested by insurance for reimbursement claims. In some cases, your insurance company might ask you to get a referral and/or an ICD-9 diagnosis code for this treatment from your doctor.
